If you become a patient, we create and maintain records containing protected health information ("PHI"), such as your diagnosis, treatment plan, and progress notes. This clinical information is handled separately from general website data and in accordance with the Health Insurance Portability and Accountability Act (HIPAA), to the extent applicable.
