01Acceptance of Terms

These Terms and Conditions ("Terms") govern your access to and use of restorativeptp.com (the "Site") and the physical therapy, performance training, and related wellness services (the "Services") offered by Restorative Physical Therapy & Performance LLC ("we," "us," or "our"). By accessing the Site, booking an appointment, or using our Services, you agree to be bound by these Terms. If you do not agree, please do not use the Site or our Services.

02Description of Services

Restorative Physical Therapy & Performance provides one-on-one physical therapy, movement assessment, and performance training for active adults and athletes, delivered in person at our Princeton, NJ location and/or through telehealth where applicable. Services are provided by licensed and/or qualified professionals in accordance with applicable state licensing requirements. No physician referral is required. Specific treatment plans are individualized and may change based on clinical judgment.

03Not a Substitute for Medical Advice

Content on the Site — including blog posts, videos, exercise demonstrations, and general wellness information — is provided for educational and informational purposes only. It is not medical advice and is not a substitute for evaluation, diagnosis, or treatment by a qualified healthcare provider. Always consult your physician or another qualified health provider before starting any new exercise or treatment program, especially if you have a pre-existing injury, medical condition, or are pregnant. Never disregard professional medical advice or delay seeking it because of something you read or viewed on the Site.

If you believe you are experiencing a medical emergency, call 911 (or your local emergency number) immediately. Do not rely on the Site or our Services for emergency medical needs.

04Assumption of Risk and Liability Waiver

Physical therapy, exercise, and performance training carry inherent risks, including but not limited to muscle soreness, strain, sprain, or other injury. By participating in our Services, you acknowledge and voluntarily assume these risks. You confirm that you have disclosed all relevant medical history, injuries, and conditions to your provider, and that you will notify your provider promptly of any pain, discomfort, or change in condition during treatment.

To the fullest extent permitted by law, you release Restorative Physical Therapy & Performance LLC, its owners, employees, and contractors from liability for injury or loss arising from your participation in the Services, except to the extent caused by our gross negligence or willful misconduct. A separate informed consent and waiver form will be provided and must be signed before your first in-person session.

05Scheduling, Cancellations & No-Shows

• Appointments may be scheduled online, by phone, or by text (732-808-3987).
• We require at least 24 hours' notice to cancel or reschedule an appointment.
• Cancellations made with less than 24 hours' notice, or missed appointments ("no-shows"), may be subject to a cancellation fee of [$XX] or forfeiture of a prepaid session.
• Repeated late cancellations or no-shows may result in a requirement to prepay for future sessions.

06Payment, Fees & Insurance

Fees for Services are due at the time of service unless other arrangements have been made in writing. We accept [list accepted payment methods]. As an out-of-network / cash-based practice [confirm this reflects your model], coverage by your health insurance is not guaranteed, and you remain responsible for any amount not covered by your plan. We can provide a superbill upon request for you to submit to your insurer for potential reimbursement. It is your responsibility to verify your insurance benefits prior to treatment. Prices are subject to change; we will make reasonable efforts to notify current clients of any changes in advance.

07Health Information & Privacy

We collect and maintain protected health information ("PHI") in accordance with applicable law, including HIPAA where applicable. Our Notice of Privacy Practices, provided separately, describes how your health information may be used and disclosed and how you can access it. Our Privacy Policy describes how we collect and use non-clinical information gathered through the Site, such as contact form submissions and analytics data.

08Use of the Site

You agree to use the Site only for lawful purposes. You may not:

• Use the Site in any way that violates applicable law or regulation.
• Attempt to gain unauthorized access to any portion of the Site or its related systems.
• Upload or transmit viruses, malicious code, or unsolicited advertising.
• Scrape, copy, or reproduce Site content for commercial use without our written permission.

09Intellectual Property

All content on the Site — including text, graphics, logos, videos, and exercise programs — is owned by or licensed to Restorative Physical Therapy & Performance LLC and is protected by copyright, trademark, and other intellectual property laws. You may view and print content for personal, non-commercial use only. Any other use requires our prior written consent.

10Testimonials and Reviews

Testimonials displayed on the Site reflect individual client experiences and results, which vary from person to person. Testimonials are used with the client's permission. Individual results are not guaranteed, and outcomes such as recovery timelines are illustrative rather than promised.

11Third-Party Links

The Site may contain links to third-party websites or services that are not owned or controlled by Restorative Physical Therapy & Performance LLC. We are not responsible for the content, privacy policies, or practices of any third-party sites.

12Limitation of Liability

To the maximum extent permitted by law, Restorative Physical Therapy & Performance LLC and its owners, employees, and contractors will not be liable for any indirect, incidental, special, or consequential damages arising out of or related to your use of the Site or the Services. Our total liability for any claim arising from the Services will not exceed the amount you paid for the specific service giving rise to the claim.

13Indemnification

You agree to indemnify and hold harmless Restorative Physical Therapy & Performance LLC and its owners, employees, and contractors from any claims, damages, or expenses (including reasonable attorneys' fees) arising from your violation of these Terms or your misuse of the Site or Services.

14Governing Law

These Terms are governed by the laws of the State of New Jersey, without regard to its conflict-of-law principles. Any dispute arising under these Terms will be resolved in the state or federal courts located in New Jersey, and you consent to the jurisdiction of those courts.

15Changes to These Terms

We may update these Terms from time to time. The effective date above reflects the most recent revision. Continued use of the Site or Services after changes are posted constitutes your acceptance of the updated Terms.
